The Queensland Government announced the Commission of Inquiry into Child Safety System on 18 May 2025 to identify and address systemic issues within the child safety system and improve the lives and outcomes for children and young people.
The Commission started on 1 July 2025.
On Thursday 12 February 2026 the Government announced it would bring forward the Commission’s reporting deadline to late May 2026 to allow reform to begin sooner.
The Commission submitted its final report to the Government on 22 May 2026.
How the Commission works
The Commission worked openly, fairly and independently.
The Commission was guided by a list of topics about the child safety system. These topics were considered during the inquiry known as the ‘terms of reference’.
As published in an Extraordinary Queensland Government Gazette on 13 February 2026, there were no changes to the Commission's terms of reference beyond bringing forward the reporting timeframes.
The Commission listened to information and stories from people all around Queensland.
The Commissioner's role was to decide what the Commission would do, how and when.
The Commission was independent from the Government. Part of the Commission’s job was to look at the way Government provided protection and support for children in Queensland. This included finding problems and suggesting solutions.

The Commission was led by The Honourable Paul Anastassiou KC, as Commissioner. The Commissioner was supported by assisting counsel and a team of people from different backgrounds, cultures, experiences and skills.
